Cardiovascular diseases, such as coronary artery disease, angina pectoris, myocardial infarction and stroke, still represent a major medical need: they are the number one cause of death world–wide with an increasing socio–economic burden. Our current lack of understanding of the underlying molecular mechanisms is reflected in our therapeutic approaches, which primarily treat symptoms of these diseases rather than their causes.

Moreover, patients at risk are identified with poor precision and the individual treatment efficacy cannot be monitored at present. Consequently, unacceptably high numbers of patients are treated to prevent even one cardiovascular death. Therefore, researchers worldwide are aiming at identifying the molecular mechanisms and therapeutic targets of vascular diseases in order to identify individuals at risk–ideally before cardiovascular symptoms become apparent–and subsequently to treat those patients to prevent disease outbreak or progression with unprecedented precision. Recent breakthroughs in identifying novel mechanisms such as endogenous nitric oxide, the Nox/Duox gene family as sources of vascular oxidative stress and a much deeper understanding of the renin– angiotensin system are exciting new developments toward that long–term objective and represent therapeutically exploitable targets.
